– Magda Jenkins keeps a record of your name, contact details, dates, times of appointments, and information provided on forms such as your health information, on a password protected system.

– You are responsible for contacting your GP or specialist about any health concerns you may have. Please advise your GP of the Nutritional Therapy protocol you will be following. Please also advise any other complementary medicine practitioners you are consulting to ensure there is no overlap.

– It is important you let Magda Jenkins know about any medical diagnosis you have received and any prescription medications, herbal or food supplements, or over the counter medication you are taking as it may affect your Nutritional Therapy plan. Magda Jenkins does not accept liability for any complications experienced as a result of your failure to disclose any medication you may be taking during your plan which may reasonably interfere or react with products recommended in your Nutritional Therapy programme.
