Consultation Agreement
1. Package Terms
- Consultation packages expire after the timeframes stated above. It is the client’s responsibility to ensure follow-up appointments are booked within this period.
- Follow-on package options are available for continued care.
2. Consultation Format
- Consultations are conducted via Zoom.
- Initial consultations may be held in person for clients based in Hampshire, UK.
- Many functional tests can now be completed from the comfort of your home.
3. Test Interpretation
- Interpretation of test results ordered by another doctor or clinic is charged at £50 per test (e.g., DUTCH, GI Effects).
- Alternatively, interpretation is charged at £50 per 15 minutes.
4. Communication & Additional Support
- Brief check-ins and quick email questions between consultations are welcome.
- More extensive support outside scheduled sessions is charged at £50 per 15 minutes.
- If more than 6 months have passed since your last appointment, a follow-up consultation is required before further advice is given.
5. Payments
- All consultations must be prepaid at the time of booking.
- Payment plans are available; please email to discuss options.
6. Cancellation Policy
- Cancellations or rescheduling must be made at least 48 hours before your appointment.
- Cancellations with less than 48 hours’ notice incur a 50% fee.
- Cancellations made within 24 hours or missed appointments incur a 100% fee.
- If you arrive late, the session will still end at the scheduled time.
7. Confidentiality
- Your identity as a client and anything discussed during sessions is strictly confidential.
- Magda Jenkins keeps a secure record of your name, contact details, appointment dates, and health information on a password-protected system.
8. Medical Responsibility
- You are responsible for informing your GP or specialist about any health concerns you may have.
- Please inform your GP of any Nutritional Therapy programme you follow.
- Inform any other complementary practitioners you are working with to avoid overlap or contraindications.
- You must disclose all medical diagnoses, prescription medications, herbal supplements, and over-the-counter products you are taking, as these may influence your plan.
- Failure to disclose medical information may result in contraindications; Magda Jenkins cannot accept liability for complications arising from undisclosed information.
9. When a Consultation Is Required
- If you develop a new condition, new symptoms, or have new test results, please book a consultation. These cannot be reviewed or interpreted via email.
10. Treatment Plan Guidance
- Your plan will include a time frame for review.
- Do not continue recommendations indefinitely; some may no longer be appropriate after the specified period (typically 2–6 weeks unless otherwise noted).
- Nutritional Therapy is highly individualised; results vary from person to person.
11. Scope of Practice
- Nutritional Therapists are not permitted to diagnose or claim to treat medical conditions.
- Nutritional Therapy is not a substitute for professional medical care.
12. Supplements & Functional Testing
- Your practitioner may recommend supplements and/or functional testing as part of your programme.
- Magda Jenkins may receive commission on certain products and services.
13. Professional Standards
- All services are delivered in accordance with the CNHC Code of Conduct and professional standards for Nutritional Therapy.
